Inspectors and operators should be aware of sub-standard maintenance practices being performed by some airline personnel. You won't believe this when you see it, but it is an actual photograph of a crack in the window frame of a US Airways DC-9. This could have caused a serious in-flight problem.
Hopefully, implementation of SMS programs will avoid re-occurrence of such an incident.
